Medical Records Request

West Coast Joint and Spine Surgery Center will only release medical and/or billing records to patients or their authorized representatives with written authorization, except as required or permitted by law.  Please allow up to 15 calendar days to receive copies of medical records after we receive a written request.  

You may request medical records by email, fax or mail.

Download and complete the authorization form.  Send the completed form by email, fax, or U.S. Mail to our facility.  

Law Firms and Copy Services 
Legal subpoenas may be served to any staff person at the Business Office.  If you utilize a copy service, requests should be mailed or hand-delivered.  All requests should include our $35.00 document production fee.  In the absence of advance payment, an invoice will be forwarded along with the requested documents.
